In a catastrophic bid of autism, I started keeping track of how my games went. Over time I started tracking more **** that I felt had a major influence on how games turned out. If we make the (rather fair) assumption that top fragging means, at the very minimum, you pulled your weight, there is a very clear pattern you have to maintain to reach S+.
The stats show me something weird though. If I have to maintain this kind of pattern to even maintain a rank of close to S 50, how are all these other people S? I rarely gets teamates (or even enemies) who manage to get 1 K/ D even during wins. It's usually a case of one person on either team doing most of the heavy lifting. Is going from A+ to S too easy? That would make the most sense, as it would explain why many people are in S yet can never manage to display any ability to even maintain it.
